: New Service, Service Revision, or Temporary Supply Requisition Form
Description :

1. This form is used for the following service applications:

1.1 - New Service
1.2 - Service Revision
1.3 - Temporary Supply

2. The form must be completed by a KAHRAMAA approved electrical contractor before it is submitted to the Installations section of the Customer Services department. The necessary attachments for each type of application are indicated at the bottom of the front page of the form.

3. After the submittal, the form is forwarded to the concerned KAHRAMAA departments for the initial approval of the application. The approval will be indicated on the bottom of the back side of the form.

4. When the approval is obtained, a copy of the form is given to the contractor to fulfill the said conditions in order to receive the requested service.

5. Any charges to be paid by the customer as a result of the required service will be received from the concerned KAHRAMAA departments on a separate letter and will be collected prior to performing any work.
